2Medical Services and Treatment
Nature of Services: Shingle Creek Medical Group provides comprehensive pain management and regenerative medicine services, including:
- Joint injection therapy for osteoarthritis and other joint conditions
- Trigger point injections for muscle tension and pain relief
- Platelet-Rich Plasma (PRP) therapy for tissue regeneration
- Wharton’s Jelly regenerative treatments (when available)
- Chiropractic care and spinal adjustments
- Non-surgical spinal decompression
- Nerve Release and Regeneration Injection Therapy (NRRIT)
- Comprehensive consultation and treatment planning
Treatment Outcomes
No Guarantee of Results: While we strive to provide effective treatments, medical outcomes cannot be guaranteed. Results may vary based on individual conditions, health status, compliance with treatment plans, and other factors beyond our control.
3Patient Responsibilities and Informed Consent
Informed Consent: Prior to receiving any treatment, patients will be provided with detailed information about:

- Potential risks, benefits, and alternatives
- Expected outcomes and recovery timeline
- Post-treatment care instructions

4Appointment Scheduling and Cancellation Policy

Cancellation Policy:
- Cancellations must be made at least 24 hours in advance
- No-shows or same-day cancellations may result in charges

- Repeated no-shows may result in termination of the doctor-patient relationship
Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule to ensure adequate treatment time and consideration for other patients.

Dispute Resolution: We encourage patients to address concerns directly with our office first. For formal disputes:
- Medical malpractice claims are subject to Minnesota’s medical malpractice statutes
- Other disputes may be subject to binding arbitration
- Venue for any legal proceedings will be in Hennepin County, Minnesota
13Updates to Terms and Conditions
